Let’s Talk About Sex, Baby! Intimacy in neurodiverse marriages can be complex, influenced by factors such as self-esteem, sensory sensitivities, physical pain, uncertainty about sexuality, lack of understanding of social norms, boundary issues, and trauma history. These challenges can create barriers to emotional and physical connection, making open conversations about sex essential. I start most sessions by asking, “What’s your sex life like?” This simple but direct question breaks the silence and allows couples to express their needs and concerns.

Why Conversations About Sex Matter in Neurodiverse Relationships
Many neurodiverse couples struggle with intimacy due to miscommunication, unmet expectations, or unspoken needs. One of the biggest barriers for autistic partners is missing subtle cues for sexual initiation. Neurotypical partners may expect their ASD spouse to “just know” when they are initiating intimacy, but ASD individuals often require direct and explicit communication to understand their partner’s desires.

Example: A neurotypical partner may touch their spouse’s arm affectionately, expecting it to be perceived as an invitation for intimacy. The ASD partner may see this as just a casual touch and miss the intended meaning, leading to feelings of rejection or frustration for both partners.
The Role of the Therapist in Neurodiverse Intimacy Work
A therapist working with neurodiverse couples must be comfortable discussing sex explicitly. Some therapists can’t, some won’t, and some don’t know how—but in neurodiverse relationships, avoiding the conversation is not an option. You don’t need a sex therapist to talk about intimacy; you need someone who understands neurodiverse communication and sensory needs.
